We are in-network with Cigna. To verify your insurance eligibility, please email us with the following information prior to your visit:

Insurance Carrier and Member ID Number
Your DOB
For out-of-network patients, we are happy to provide you with an itemized receipt, called a Superbill, which contains all the information you need to get reimbursed directly by insurance. Just request it at the time of your visit!

Cancellation Policy

Please provide 24-hours notice to cancel or reschedule your appointment, or you will be charged the full fee for the missed visit. Thank you!
